Your friends in Memphis can link you to care early!

At Friends For All, we have Early Intervention Services (EIS) that can assist our Friends/clients in the Mid-South. We help our clients in their first steps to receiving care. Scroll below to find out more about what our Early Intervention Services and what our team does for our Mid-South Friends.

linkage to care

  • Our goal is to support the Mid-South through:

    • Being a point of entry for Friends affected by HIV and our neighbors

    • Identifying and addressing Social Drivers of Health that prevent Friends from living well

    • Supporting Friends journey through the continuum of care

  • We connect newly diagnosed clients and clients who are out of care to HIV providers.

  • We give handoffs to an agency medical case manager.

  • We focus on communicating the importance of taking medication, following a diet, and executing lifestyle changes to encourage clients to obtain an undetectable viral load and best quality of life possible.

  • We provide clients with information on partner agencies that provide HIV testing free of cost.

  • We perform an assessment used to rate the client’s vulnerability and need level to be placed on a waitlist for possible housing options.

  • We help clients get connected to outside resources through a referral process.

The EIS team offers support, care, knowledge, and wisdom for clients who are newly diagnosed, being released from incarceration, in care, but have identified issues that adversely impact retention in care or out of care. Our mission is to create a bridge for Friends to cross over the barriers of fear, social stigma, and unmet needs to a place of empowerment, self-sufficiency, and acceptance.
