Linkage to care

The Early Intervention Services (EIS) team offers support, care, knowledge, and wisdom for clients who are newly diagnosed, being released from incarceration, or in care, but have identified issues that adversely impact retention in care or out of care. Our EIS team is often your first contact when obtaining services from Friends For All. Learn more about our EIS team by clicking one of the drop-down options below.

  • Our goal is to support the Mid-South through:  

    • Being a point of entry for community members/Friends affected by HIV

    • Identifying and addressing Social Drivers of Health that prevent our Friends from living well 

    • Supporting our Friends’ journey through the continuum of care 

  • We connect newly diagnosed clients and clients who are out of care to HIV providers.

  • We give handoffs to an agency medical case manager. 

  • We focus on communicating the importance of taking medication, following a diet, and executing lifestyle changes to encourage clients to obtain an undetectable viral load and the best quality of life possible.

  • We provide clients with information on partner agencies that provide HIV testing free of cost.

  • We perform an assessment used to rate the client’s vulnerability and need level to be placed on a waitlist for possible housing options.

  • We help clients get connected to outside resources through a referral process.

When you visit Friends For All for the first time, you will meet an EIS team member who will help assess your needs and get you connected to programs and resources. To do that, you’ll need a few documents:  

  • Proof of diagnosis 

  • Proof of residency (within the last 60 days) 

  • Proof of income  

  • Photo ID 

  • Proof of insurance OR lack of
