Medical case Management

Our case managers work to build relationships with people living with HIV. We focus on identifying strengths and skills to develop goals for treatment.   • We connect newly diagnosed clients and clients who are out of care to HIV providers. 

  • We focus on communicating the importance of taking medication, following a diet, and executing lifestyle changes to encourage clients to obtain an undetectable viral load and the best quality of life possible.

  • We help uninsured clients obtain medication at no cost. 

  • We enroll clients in insurance assistance programs that provide financial support by paying premiums, copays, and deductibles. This helps clients maintain reliable medical insurance.

  • We help clients get connected to outside resources through a referral process. 

To work with a Medical Case Manager at Friends For All, you must have:

  • Proof of diagnosis

  • Proof of residency (within the last 60 days)

  • Proof of income

  • Photo ID

  • Proof of insurance OR lack of
